Peel Case Management (PCM)
Service Coordination This program is 100% funded by the Ministry of Community and Social Services and the Ministry of Children and Youth Services. It provides service coordination and assistance with accessing community participation supports for children and young adults who have a developmental disability and their families.
Who is Eligible?
- Children and young adults who have a diagnosed developmental disability (E.g. Fragile X, Down Syndrome, Rett Syndrome, Global Developmental Delay) and their families
- This includes those who have Autism Spectrum Disorder and/or a Dual Diagnosis (developmental disability with a mental health need).
- Youth (fifteen years old +) who meet above criteria and are wards of the Children’s Aid Society.
- Children and young adults, who have learning disabilities, ADD, ADHD (Attention Deficit Disorder with Hyper Activity), Tourette syndrome, speech and language delays, and physical or behavioral conditions, must also have a diagnosis of a developmental disability to be eligible for service.
What Services are Available?
All eligible families and individuals have access to services at the point of intake. Two levels of service are available to meet immediate and long term needs. This is determined by completing a comprehensive individual and family assessment and may change based on the needs at any point in time during service provision. Services are strength based and focus on capacity and skill building. Contact is typically initiated by family and is parent directed.
Level One – Resource Coordination
- Provides periodic access to information, advice and service system clarification through one-to-one contact, information resources and outreach sessions.
- Supports families to identify their needs and determine their most effective solution.
- Provides anticipatory guidance specific to long term planning.
- Assists families to explore, build and maintain a wide range of formal and informal supports.
Level Two - Intensive Service Coordination
- Provides concentrated service for individuals/families experiencing crisis, significant stress and/ or difficulty coping with complex needs through telephone counseling and home visits.
- A Family Resource Worker assists the individual/ family to:

- Complete a comprehensive assessment and develop a detailed service plan.
- Explore, build and maintain a wide range of formal and informal supports.
- Coordinate services, liaise with other professionals, organize case conferences, clarify roles, and complete applications for programs and/or funding.
- Monitor and evaluate service provision to maximize effectiveness and prevent duplication.
